My Health for Life is a free healthy lifestyle program for Queenslanders, funded by the State Government and delivered by an alliance of not-for-profit agencies. 

We are partnered with Diabetes Queensland, the Heart Foundation, the Stoke Foundation, the Queensland Aboriginal and Torres Strait Islander Health Service, the Ethnic Communities Council of Queensland and the (QLD) Primary Health Networks working together with local health providers to improve the health of Queenslanders.

What the program offers

What to expect

Session One
We begin with a one-on-one session which will get the ball rolling. In this session we tailor your program and build a strategy around your needs and goals.
Session Two - Five
We all come together as a small group for 8 weeks (1 hour per week) or 4 fortnights (2 hours per fortnight) and share any barriers we may be facing. In these sessions we provide more resources, tips and strategies to help you achieve your goals.
4 week break
Session Six
The final session is our last group session, but that doesn't mean we stop. We support each individual in making sure a plan is in place to continue smashing your goals.
8 week break
Session Seven
We want you to achieve you goals so our session 7 is a telephone catch-up call to check in and see how you are tracking.
